We want your first visit to be a pleasant, stress-free experience. We have provided these forms for you to complete before your first visit. If you prefer to wait until you arrive to complete the paperwork, that’s fine too! We’ll just need you to arrive a little earlier so you can see your therapist at your scheduled appointment time.
